How do I get a copy of my Birth Certificate?

Note: The Town of Babylon Clerk’s office will only have a copy of your Birth Certificate if you were born in the Town of Babylon. See attached list of Towns and Villages. If you were born in Brunswick Hospital, please contact the Village of Amityville at 631-264-6000. Only the person named on the Birth Certificate if 18 years of age or older, the parents listed on the Birth Certificate or a lawful representative of the person named or the parents of the person named on the Birth Certificate. 

A. In order to obtain a certified copy of your birth certificate by mail, you will need:

  1. Fill out the attached form: (Birth Certificate Request Application)
  2. A notarized letter requesting a copy of your birth certificate which must include both your parent’s names with your mother’s maiden name and your date of birth.
  3. Copy of your driver’s license or non-driver’s license
  4. Money order made out to the Town of Babylon on the amount of $10 per copy.
  5. Mail your request to:
